Application Process
Check Eligibility Status
If you don't have the relevant private
health insurance or access to sufficient
funds, SuperCare can help. You only
need one member with Superannuation
funds to be eligible for dental treatment.
Online Referral Form
Completion
Through an online referral form,
Hills Family Dental Centre sends
SuperCare your patient information.
Fill out the form with your contact
information and personal information.
SuperCare Eligibility
Consultation
Using the contact information you
have provided, a SuperCare
consultant will contact you.
SuperCare can be contacted
directly at 1300 665 440 if you wish
to inquire about the status of your
referral.
Compassionate Release of
Super (CRS) Application
Process
You can then begin the CRS application
process once you have been signed up
as a client. A supporting letter will be
requested by SuperCare from the dental
clinic in order to process the CRS claim.
It is also required that a general
practitioner sign the CRS form.
ATO Assessment &
Approval
It can take up to 10 business days
for the ATO to assess an application
after it has been lodged with them.
As soon as the application is approved,
our clinic is notified. You can expect the
funds to be released in 5-7 business
days.
